Blocking the NOTCH Pathway Inhibits Vascular Inflammation in Large-Vessel Vasculitis

Piggott K et al. – Blocking NOTCH signaling depleted T cells from the vascular infiltrates, implicating NOTCH–NOTCH ligand interactions in regulating T–cell retention and survival in vessel wall inflammation. Modulating the NOTCH signaling cascade emerges as a promising new strategy for immunosuppressive therapy of large–vessel vasculitis.

Treatment options for large vessel vasculitis are limited, mostly relying on corticosteroids. Alternative therapies are urgently needed. This study has explored which receptor-ligand pairs are involved in driving the inflammatory responses in the arterial wall. Temporal artery biopsy specimens from patients with giant cell arteritis expressed high levels of the NOTCH receptor and its ligands. In humanized mice blocking the NOTCH pathway successfully suppressed the early and the established phase of vessel wall inflammation. NOTCH blockade markedly impaired recruitment and retention of T cells in the inflammatory infiltrates. Inhibiting the accumulation of disease-promoting lymphocytes in the vascular infiltrates may represent a novel mechanism of treating vasculitis.
